I confirm I am a UK taxpayer and would like the University of Leeds to treat all donations I have made during the previous four years and all donations I make in the future as Gift Aid donations, until I notify you otherwise. I have paid sufficient income tax or capital gains tax on all my donations to charities to cover the tax that you will be reclaiming on my donations for each tax year (6 April to 5 April), and I understand if this is not the case, it is my responsibility to pay any difference.

If an error is made in the payment of your Direct Debit, by Charities Aid Foundation (CAF) or your bank or building society, you are entitled to a full and immediate refund of the amount paid from your bank or building society
If you receive a refund you are not entitled to, you must pay it back when Charities Aid Foundation (CAF) asks you to.
